Practice policies
How this practice runs
Qualifications, privacy, cancellations, fees and consent, set out plainly. You will sign the full intake and consent form before your first appointment; this page is so you can read the terms before you get there.
Qualifications
My legal name is Ariel Barook; I practise as Ari. I am a Certified Athletic Therapist with the Canadian Athletic Therapists Association (registration number 2-7373) and a Registered Massage Therapist with the College of Massage Therapists of Ontario (registration number B-4945). I hold a B.A. in Kinesiology, a B.A. in Athletic Therapy and a Diploma in Massage Therapy, and I am a certified First Responder.
I take part in ongoing professional development, and I consult with other practitioners to discuss cases confidentially and anonymously. Training, courses and independent learning are part of how I keep improving the care I give.
Athletic Therapy and Massage Therapy are healthcare professions specialising in the prevention, assessment and rehabilitation of musculoskeletal injuries.
Privacy, confidentiality and your information
Your personal and medical information is confidential. It is collected so that I can assess and treat you properly, and it is only disclosed to a third party with your permission.
Where it would benefit your treatment, I may need to communicate with your family doctor or a referring practitioner. You authorise that as part of the intake form, and you can withdraw it at any time.
Cancellation policy
Your appointment time is reserved for you. A late cancellation or a missed visit leaves a gap in the day that another patient could have used.
If you cannot make your appointment and have not given at least 24 hours’ notice, it is treated as a no-show and the full cost of the session as booked is payable.
To cancel or move an appointment, contact me at least 24 hours beforehand by email or text. I know that some things are genuinely outside your control, such as sudden illness or a family emergency, so tell me even if it is last minute.
If I am unwell or have an emergency, I will let you know as soon as I can and reschedule you promptly for the next time I have available. You will not be charged for an appointment missed at my end.
Fees and payment
Payment is by e-transfer or cash at this time, and is due at the end of each session. E-transfers go to aribarookrehabilitation@gmail.com.
If you have insurance coverage for Athletic Therapy, CAT(C) or RMT services, pay the fee by e-transfer and I will give you a receipt to submit to your insurer for reimbursement.
Please check your coverage before you book. Clients are responsible for confirming that their plan covers Athletic Therapy specifically. If Athletic Therapy is not named on your plan and payment has already gone through after the session, refunds are not available. Note that “Sport Therapy” is not the same as “Athletic Therapy”, and insurers may not reimburse a receipt labelled Athletic Therapy against a Sport Therapy benefit.
Current rates are on the services and pricing page, and include HST.
Risks and informed consent
Athletic Therapy and Massage Therapy involve physical activity and hands-on treatment. Before treatment begins you will have the chance to discuss what is involved, and results cannot be guaranteed.
Both are generally safe, but there are potential risks:
- Temporary discomfort, bruising, or muscle, joint or bone soreness
- Aggravating an existing injury
- An allergic reaction to tape or treatment materials, such as pain relief gel, oil, lotion or latex gloves
- An unexpected outcome despite following the rehabilitation plan
- Light-headedness or dizziness
You will be draped at all times, and any area left undraped will be secured so there is no indecent exposure.
You can stop or change treatment at any point. You may withdraw consent and discontinue treatment at any time, for any reason, without needing to explain. If Massage Therapy is not the right route for you, alternatives include consulting other healthcare professionals.
By signing the intake form, you acknowledge these risks and release the therapist (Ariel Barook) from liability for injuries or adverse reactions, other than in cases of gross negligence.
